A transplant moves hair. It does not create it, and it does nothing for the hair you still have that is on its way out. If your follicles are dormant — thinning, producing weaker strands, but still alive — then transplanting past them ignores the problem. Those areas keep receding behind your new grafts, and a few years later the result looks strange and you are back for more surgery.
